Data quality survey template

Prior to investing in data quality initiatives, you may want to conduct a data quality survey. Here's a template for such a survey that can be filled in for your own use.

Note that this survey tries to get both a qualitative and quantitative sense of the state of data at your company. It should be distributed to all producers and consumers of data at the company, in particular, data, revenue, finance, marketing teams.
